2005-2006 SCHOLAR-ATHLETE AWARD

The purpose of this award is to emphasize the importance of scholastic achievement through a program that recognizes the scholastic excellence of Potomac Valley swimmers.

ELIGIBILITY: Open to all PVS-USA Swimming athletes who:
  1. have completed grade 9, 10, 11 or 12 during 2005-2006,
  2. have attained a 3.5 or higher grade point average (GPA) for the ENTIRE 2005-2006 school year, AND
  3. have achieved a 3.5, or higher, event standard average in a minimum of five events.
Note: The above wording has been updated to clarify current PVS requirements for receiving this award.

 

APPLICATION:

A student must submit an application. A copy of the student’s final report card must be included with the application.

YEAR END REPORT CARD IS REQUIRED FOR VERIFICATION OF CUMULATIVE GPA.

SIGNATURE OF GUIDANCE COUNSELOR IS REQUIRED ONLY IF YOUR SCHOOL DOES NOT ISSUE REPORT CARDS WITH A CALCULATED CUMULATIVE GPA.

DEADLINE:

All applications with (1.) report cards, AND (2.) Counselor’s signature (if school does not issue report cards with a calculated cumulative GPA), MUST BE POSTMARKED AND/OR RECEIVED NO LATER THAN AUGUST 15, 2006.

A swimmer must also maintain a 3.5, or higher, event standard average in a MINIMUM OF FIVE EVENTS offered in the swimmer's age group. Times must be obtained at a USA Swimming or Potomac Valley Swimming sanctioned or approved short course or long course meet during the 2005-2006 season. High school dual-meet times may not be used. (You may use the time standards applicable to your age on the first day of the meet.)

Please list each event, time, and event standard.

You may find a copy of your times and time standard achieved from the USA Swimming website. Only request times for the time period 2006 (top 16). You may print out a copy and attach -- highlighting the five swims you want considered.

Conversion of letter grades/event standards to point values are as follows:

Application for this award must be made on the form provided. Completed applications may be sent through your coach (signature required), or directly to the address below. Direct submission is highly recommended.

NO APPLICATIONS WILL BE ACCEPTED AFTER AUGUST 15, 2006. Incomplete applications will not be processed. Award recipients will be notified by mail. Please keep a copy of your application.
